Request Forms is a form builder at requestforms.io. The search name is requestforms. You describe questions, rules, and flow in plain language. An in-app agent writes a real form in this product. You can also build field by field in the visual editor. Publish a hosted link or embed the form with a web component. Answers land in your response inbox.
The product is for people who collect requests, applications, RSVPs, payments, and feedback. It is not a spreadsheet. It is not a survey panel. You can export a new Google Sheet from Responses. Stay on Google Forms if the sheet must update on every submit. Stay on Fillout if you need native Notion, Airtable, or piping.
Most of the builder stays on the free plan, subject to rate limits. Paid is $23.20 per month billed yearly (20% off), or $29 per month. The paid features are hiding Request Forms branding and a custom domain. Session cookies power the website and the MCP server. API keys are a free Bearer token that starts with rfk_.
