How to use it

  1. Open the Google Forms comparison for the full table.
  2. Rebuild the questions in Request Forms, or describe them to the agent.
  3. Add payment, signature, or CSS if those were the gaps.
  4. Export CSV or a new Google Sheet if you still want a spreadsheet archive.

Will answers go to Google Sheets?
Export to Google Sheets from Responses. That creates a new spreadsheet. It does not keep updating. Stay on Google Forms if the sheet must update on every submit.
Is Request Forms free like Google Forms?
Most features stay free, including responses. Paid is hide branding and a custom domain. See Pricing.
