Open Write a form and describe the quiz, or insert Multiple choice and Rating blocks.
Add Number answers or mapped scores, then insert a Calculated field for the total.
Add logic if a low score should show a follow-up question.
Publish the hosted link or embed the form. Read scores in Responses or export CSV.
