OAuth

  1. Add https://requestforms.io/mcp in the MCP client.
  2. Sign in on /oauth/authorize with email or Google.
  3. Allow the client to manage forms on that account.

Claude hosted apps use https://claude.ai/api/mcp/auth_callback. Claude Code uses localhost loopback redirects. Discovery documents live at /.well-known/oauth-authorization-server and /.well-known/oauth-protected-resource/mcp.
